Prime Care is an initiative to establish coordinated and integrated primary and behavioral health care services in Detroit, MI.The target population is low-income African American Detroit-Wayne County residents with serious mental illness (SMI) or SMI with Co-Occurring Disorder and who have or are at risk of co-morbid physical health problems including chronic disease.An integrated Treatment team will be established, with the primary care provider and psychiatric provider participating as an integral part of the team.
The population of focus is adults with SPMI and adults with Co-occurring SUD of Allegan County.
This population is more racially diverse (32% nonwhite), older (average 43 years old), and poorer than the average county citizen, with a higher incidence of hypertension, diabetes, smoking and obesity.
The catchment area is Allegan County located in the heart of West Michigan on the Lake Michigan shore.
We will target adults 18-years-old and older with Serious Mental Illness in Kalamazoo County MI, eligible for/in need of KCMHSAS Psychiatric Services (1000 consumers).
The project will prioritize enrolling people who do not currently have a Primary Care provider (32% of the Psychiatric Clinic consumers).
